Private Training and Special Event Agreement - Premier Jiu Jitsu Academy

This Agreement is made by and between the individual engaging in private training sessions, special events, or seminars (hereinafter referred to as "the Participant") and Premier Jiu Jitsu Academy, located at 1587 Lee St., Des Plaines, IL 60018 (hereinafter referred to as "the Academy").

1. Non-Refundable Payment: The Participant acknowledges that all payments made for private training sessions, special events, or seminars are non-refundable under any circumstances.

2. Scheduling and Rescheduling: The Participant understands that the scheduling of private training sessions, special events, or seminars is subject to the availability of instructors and facilities at the Academy. Rescheduling is permitted at the discretion of the assigned instructor or event coordinator and must not conflict with the Academy's schedule of ongoing classes, other private lessons, or events. The Academy does not guarantee the availability of alternative time slots for rescheduled sessions or events.

3. Liability Release: The Participant agrees to release, indemnify, and hold harmless the Academy, its officers, employees, instructors, agents, and affiliates from any claims, demands, losses, actions, damages, injuries, and liabilities of any kind related to the Participant's involvement in private training sessions, special events, or seminars. This includes but is not limited to, personal injury, financial loss, and dissatisfaction with the services provided.

4. Acknowledgment of Risks: The Participant acknowledges that martial arts, fitness activities, and participation in special events or seminars carry inherent risks, including but not limited to physical injury. The Participant assumes all risks associated with participation in any activities conducted during private training sessions, special events, or seminars.

5. Agreement to Follow Rules and Instructions: The Participant agrees to adhere to all rules and regulations set forth by the Academy and to follow the instructions of the instructors or event coordinators during private training sessions, special events, or seminars. Non-compliance may result in the termination of participation without refund.

6. Entire Agreement: This document constitutes the entire agreement between the Participant and the Academy concerning private training sessions, special events, or seminars and supersedes all prior agreements and understandings, whether written or oral.

By engaging in private training sessions, special events, or seminars at Premier Jiu Jitsu Academy, the Participant acknowledges having read, understood and agreed to the terms and conditions outlined in this Agreement. This Agreement is legally binding from the date of the first scheduled session, event, or seminar.
